Recalling Firm: AmeriSource Health Services Corp., Columbus, OH, by letters on February 19, 2003.
Manufacturer: Bristol-Myers Squibb Co., Princeton, NJ. Firm initiated recall is ongoing.

REASON
Dissolution Failure: by manufacturer (Bristol-Myers Squibb).

VOLUME OF PRODUCT IN COMMERCE
5,000 bottles.
